Goal setting: For every patient individual goals are set with the timeline

Plan of care: In order for the goals to be achieved a plan is formulated, care to be delivered by personnel’s on a shift basis /visit basis.

Visit based: would entail a doctor/ therapist/nursing staff visiting the patient and carrying out interventions. A shift can last for anywhere between 2-4 hours.

Shift based: For patients who are critically sick/bed ridden or are unable to function without support a shift based care is given. The nursing attendant or the staff nurse would provide services to the patient in shifts lasting 8-12 hours.

Monitoring/supervisory visits: by clinical team leads to ensure the staff is functioning and delivering care to their full potential and evaluating the outcomes of the care that is delivered.
